Patient Centered Medical Home

Eastlake Pediatrics is a designated “Patient Centered Medical Home.” We have an ongoing commitment to partner with each of our families in order to provide exceptional care. This partnership contains responsibilities for us, your health care providers, and you, our families.

Responsibility of Your Primary Care Provider

Work with each patient and family to promote and improve good health and wellbeing
Medications are reviewed and managed at each visit
Develop a plan of care to manage any health problem, including chronic health problems
Test results are given to the parents/caregivers in a timely manner
Educational material and information about community resources are available and distributed
24 hour phone access is provided to contact a medically trained professional
Work with emergency and after hour urgent care facilities to ensure that we are informed when our patients are seen
Obtain consult reports from any specialist in a timely manner to ensure consistency of care
Offer same day sick appointments

Responsibility of Patient Families

Provide all medical history, medications and allergies to health care provider
Participate with the health care provider to create a comprehensive health care plan
Make sure all scheduled appointments are met
Follow all recommendations by health care provider
If needed, ask for a clearer explanation from health care provider
Ask for guidance from the health care provider before making an appointment with a specialist
Provide feedback as to how our office can possibly improve patient care
